GRHC SPECIAL DIABETES PROGRAM FOR INDIANS - THE GILA RIVER INDIAN COMMUNITY (GRIC) IS LOCATED ON 372,000 ACRES (APPROXIMATELY 600 SQUARE MILES), 25 MILES SOUTH OF PHOENIX, ARIZONA. GRIC MEMBERS ARE COMPRISED OF TWO DISTINCT PEOPLES: THE AKIMEL O’ODHAM—THE RIVER PEOPLE—(PIMA INDIANS) AND THE PEE POSH (MARICOPA INDIANS). THE GRIC MEMBERS RESIDE BOTH WITHIN SEVEN (7) DISTINCT COMMUNITY DISTRICTS AS WELL AS OUTSIDE OF THE COMMUNITY BOUNDARIES IN LOCAL URBAN AREAS. THE DIABETES PREVALENCE RATE AMONG GRIC PATIENTS IS 25.3%, COMPARED TO 10.7% OF ARIZONANS (GRHC NEXTGEN; AMERICAN DIABETES ASSOCIATION, THE BURDEN OF DIABETES IN ARIZONA, 10/2021). BY RESOLUTION, THE GILA RIVER INDIAN COMMUNITY AUTHORIZES THE GILA RIVER HEALTH CARE CORPORATION TO APPLY AS PRIMARY GRANTEE FOR THE SDPI GRANT ON BEHALF OF THE COMMUNITY. AS SUCH, THE PURPOSE OF THE GILA RIVER SDPI IS TO PROVIDE SERVICES TO GRHC PATIENTS AND GRIC MEMBERS IN ORDER TO REDUCE DIABETES ONSET, REDUCE RISK FACTORS ASSOCIATED WITH DIABETES, AND ELIMINATE AND/OR REDUCE COMPLICATIONS ASSOCIATED WITH DIABETES. THERE ARE FOUR HEALTH CARE FACILITIES OPERATED BY GRHC: HU HU KAM MEMORIAL HOSPITAL, KOMATKE HEALTH CENTER, RED TAIL HAWK HEALTH CENTER, AND AK-CHIN CLINIC IN THE PROVISION OF CLINICAL-BASED SERVICES. THE GILA RIVER SDPI WORKS COLLABORATIVELY WITH ALL FOUR FACILITIES. CLINICAL SDPI SERVICES ARE CLOSELY LINKED TO THE GRHC MEDICAL PROVIDERS AND ACTIVE REFERRAL SYSTEMS ARE USED DAILY FOR SDPI SERVICES. THE GILA RIVER SDPI SUPPORTS AND COLLABORATES WITH TWO DISTINCT TRIBAL PARTNERS: THE GENESIS PROGRAM UNDER THE TRIBAL HEALTH DEPARTMENT AND TRIBAL RECREATION AND WELLNESS UNDER THE COMMUNITY SERVICE DEPARTMENT IN THE PROVISION OF COMMUNITY–BASED SERVICES. ACTIVE PARTNERSHIPS BETWEEN SDPI STAFF, GRHC, AND TRIBAL PARTNERS ARE CRUCIAL ELEMENTS TO EFFECTIVE PATIENT RECRUITMENT AND RETENTION. THIS GILA RIVER SDPI HAS SELECTED THE BEST PRACTICE OF CHRONIC KIDNEY DISEASE SCREENING AND MONITORING AND WILL MONITOR THE REQUIRED KEY MEASURES RELATED TO THIS BEST PRACTICE ON THE SELECTED TARGET POPULATION. IN ADDITION TO THE BEST PRACTICE, ACTIVITIES WILL INCLUDE: OTHER CLINICAL ACTIVITIES NOT RELATED TO THE BEST PRACTICE SUCH AS BLOOD PRESSURE AND DIABETES–RELATED EDUCATION AND COMMUNITY-BASED SERVICES FOCUSED ON MATERNAL AND NEWBORN NUTRITION, SCHOOL-AGED NUTRITION AND PHYSICAL ACTIVITY AND FITNESS ACTIVITIES INCLUDING ALL AGES FROM YOUTH THROUGH ELDERS.

SAN CARLOS APACHE HEALTHCARE CORPORATION DIABETES TREATMENT AND PREVENTION PROGRAM - THE SAN CARLOS APACHE INDIAN RESERVATION IS A 1.8-MILLION-ACRE LAND BASE OF THE FEDERALLY RECOGNIZED SAN CARLOS APACHE TRIBE LOCATED IN SOUTHEASTERN ARIZONA. WE ARE A RURAL ARIZONA TRIBE, LOCATED 90 MILES EAST OF THE METROPOLITAN PHOENIX AREA. THE SAN CARLOS APACHE TRIBE HAS APPROXIMATELY 17,050 ENROLLED MEMBERS. IN ADDITION TO TYPE 2 DIABETES, THERE ARE OTHER DIABETES-RELATED COMORBIDITIES THAT ARE PREVALENT AMONG THE SAN CARLOS APACHE TRIBE SUCH AS OBESITY, CARDIOVASCULAR AND KIDNEY DISEASE. THE SAN CARLOS APACHE HEALTHCARE CORPORATION (“SCAHC”) IS A SUBSIDIARY NONPROFIT HEALTHCARE ENTERPRISE OF THE SAN CARLOS APACHE TRIBE ESTABLISHED FOR THE PURPOSE OF PROVIDING DIRECT HOSPITAL AND CLINICAL SERVICES. SCAHC HAS BEEN WORKING ON BUILDING A COMPREHENSIVE DIABETES TREATMENT PROGRAM SPECIFIC TO THE SAN CARLOS APACHE TRIBE ALONGSIDE THE COMMUNITY DRIVEN DIABETES PREVENTION EFFORTS. CONTINUOUS EFFORTS BY SDPI COUPLED WITH SCAHC FUNDS WILL BE A SIGNIFICANT STEP IN ADDRESSING DIABETES AND ITS COMORBIDITIES ON THE SAN CARLOS APACHE RESERVATION. THE SAN CARLOS APACHE TRIBE HAS IDENTIFIED AN INCREASE IN DISEASE BURDEN SINCE THE BEGINNING OF THE CURRENT GRANT CYCLE FY2016-FY2022. IN 2015, 1392 INDIVIDUALS WITH DIABETES WERE RECEIVING THEIR CARE IN SAN CARLOS. IN MARCH 2020 PRIOR TO THE PANDEMIC, WE IDENTIFIED 1743 INDIVIDUALS WITH DIABETES. DURING THE PANDEMIC, AS MORE INDIVIDUALS BEGAN COMING IN TO SCAHC FOR ACUTE ILLNESS INCLUDING COVID INFECTIONS, THE HOSPITAL BEGAN TO IDENTIFY UNDIAGNOSED INDIVIDUALS WITH UNCONTROLLED DIABETES. AS OF THIS YEAR, WE NOW HAVE OVER 1900 COMMUNITY MEMBERS WITH DIABETES THAT ARE ACTIVELY RECEIVING THEIR CARE AT SCAHC AND AN ADDITIONAL 675 INDIVIDUALS THAT ARE DIAGNOSED WITH DIABETES THAT ARE NOT ACTIVELY RECEIVING THEIR CARE AT SCAHC. IN OUR EFFORTS TO IDENTIFY PREDIABETES AND DIABETES, SCAHC HAS TAKEN THE INITIATIVE OF COMPLETING GLUCOSE CHECKS WHEN PATIENTS ARRIVE FOR ANY APPOINTMENT. SCAHC HAS ALSO ADDED A MOBILE HEALTH UNIT TO ASSIST IN PROVIDING HEALTHCARE WITHIN THE COMMUNITY TO INDIVIDUALS THAT MAY NOT HAVE OTHERWISE RECEIVED HEALTHCARE. WITH THE ADDITION OF THIS MOBILE HEALTH UNIT, OUR HOPE IS THAT WE CAN BUILD TRUST WITHIN THE COMMUNITY, PROVIDE HEALTHCARE AND EDUCATION TO PREVENT THE ONSET OF DIABETES AND MANAGE DIABETES TO DECREASE THE RISK OF DIABETES-RELATED COMORBIDITIES. WITHIN OUR SDPI CLINIC, WE CONTINUE TO PROVIDE DIABETES EDUCATION AND MEDICAL NUTRITION THERAPY AS WELL AS COMPREHENSIVE DIABETES CARE BY NURSE PRACTITIONERS, ENDOCRINOLOGISTS, A PODIATRIST, OPTOMETRISTS, DENTISTS, AND OUR PRIMARY CARE TEAM. THROUGH THIS COMPREHENSIVE CARE APPROACH OUR GOAL IS TO BUILD LONG LASTING TRUST WITH THE COMMUNITY MEMBERS AND IN RETURN IMPROVE GLYCEMIC CONTROL AND DECREASE RISK OF DIABETES AND ALL ITS COMORBIDITIES. AS OF SEPTEMBER 2021, WE HAVE ALSO BUILT A 3-TIER COMPREHENSIVE WEIGHT LOSS PROGRAM INCLUDING TIER 1 WITH NUTRITION AND LIFESTYLE MODIFICATIONS ONLY, TIER 2 ADDING MEDICATION ASSISTED WEIGHT LOSS AND TIER 3 COMPLETING BARIATRIC SURGERY WHEN TIER 1 AND 2 HAVE FAILED. SINCE THEN, WE HAVE ~80 INDIVIDUALS THAT ARE UTILIZING OUR MEDICATION ASSISTED WEIGHT LOSS AND 21 INDIVIDUALS THAT HAVE UNDERGONE BARIATRIC/METABOLIC SURGERY. MANY INDIVIDUALS HAVE BEEN ABLE TO REDUCE OR COMPLETELY STOP THEIR CHRONIC MEDICATIONS. SAN CARLOS HAS ALSO IDENTIFIED ~2225 INDIVIDUALS WITHIN THE COMMUNITY THAT HAVE PREDIABETES WHO WILL HAVE THE POSSIBILITY OF IMPROVING THEIR HEALTH THROUGH LIFESTYLE MODIFICATIONS AND DECREASING THEIR WEIGHT THROUGH COMMUNITY OUTREACH, THE DIABETES PREVENTION PROGRAM, AND EARLY INTERVENTION OF DIETITIAN SUPPORT. THE SDPI GRANT WOULD CONTINUE TO ASSIST IN GROWING OUR CURRENT AND NEW PROGRAMS. ADDITIONAL FUNDING TO BE CONSIDERED WOULD ALSO ASSIST IN OUR COMMUNITY OUTREACH EFFORTS THROUGH IMPROVEMENTS TO OUR FITNESS CENTERS INCLUDING GROUP FITNESS ROOMS AND CLASSROOMS TO PROVIDE A COMMUNITY-BASED DIABETES PREVENTION PROGRAM.

FAMILIES MUST BOIL WATER OR FIND ALTERNATIVE MEANS TO MEET BASIC POTABLE WATER NEEDS FOR DRINKING, COOKING, WASHING CLEANING, AND SANITATION HYGIENE - SOMETIMES TRAVELING MANY MILES FROM HOME TO DO SO. PLANNING AND DESIGN WILL ANALYZE ALTERNATIVES AND DESIGN A WATER DELIVERY SYSTEM TO CONVEY WATER FROM AN EXISTING NEARBY RESERVOIR TO NEW TREATMENT FACILITIES FOR SUPPLY TO THESE UNDERSERVED HOMES, PROVIDING MORE RELIABLE ACCESS TO SAFE, CLEAN DRINKING WATER. LOCATED AT 150 N. 18TH AVENUE, PHOENIX, ARIZONA, 85007, THE AZDHS WILL IMPLEMENT SPAN ACTIVITIES THROUGHOUT THE STATE OF ARIZONA AND IN THE GEOGRAPHIC AREA OF FOCUS IN YUMA COUNTY, ARIZONA. CHRONIC DISEASES AND THEIR ASSOCIATED RISK FACTORS DISPROPORTIONATELY AFFECT RACIAL AND ETHNIC MINORITY GROUPS ACROSS THE U.S., AND ARIZONA IS NOT EXEMPT FROM THESE CHALLENGES. PEOPLE OF HISPANIC OR LATINO ORIGIN IN ARIZONA EXPERIENCE INCOME AND HEALTH INEQUITIES, INCLUDING HIGHER RATES OF OBESITY, DIABETES, AND HYPERTENSION. ADDITIONALLY, THE NEED FOR TRAUMA-INFORMED APPROACHES TO STATEWIDE NUTRITION, PHYSICAL ACTIVITY AND BREASTFEEDING PROGRAMS IS AT AN ALL-TIME HIGH, AS AMERICANS EMERGE FROM THE COVID-19 PANDEMIC AND THE 2022 INFANT FORMULA SHORTAGE. THIS PROJECT PROVIDES AZDHS WITH AN OPPORTUNITY TO ADDRESS NUTRITION AND PHYSICAL ACTIVITY FACTORS BEYOND INDIVIDUAL BEHAVIORS AND IMPACT SYSTEMS’ CHANGE TO REDUCE THESE DISPARITIES. WITH SUPPORT FROM THE CDC_RFA-DP-23-0012, THE AZDHS BUREAU OF NUTRITION AND PHYSICAL ACTIVITY (BNPA), OFFICE OF COMMUNITY INNOVATIONS (OCI), WILL IMPLEMENT A FIVE-YEAR (APPROXIMATELY $1,175,000 ANNUALLY) ARIZONA STATE PHYSICAL ACTIVITY AND NUTRITION PROGRAM (AZSPAN). AZSPAN WILL EXPAND THE REACH OF AZDHS’S TRAUMA-INFORMED APPROACHES TO NUTRITION AND PHYSICAL ACTIVITY TO SHAPE POLICIES, SYSTEMS, AND ENVIRONMENTS THAT SUPPORT POSITIVE BEHAVIOR CHANGE AND REDUCE HEALTH DISPARITIES. THIS WORK ALIGNS WITH THE ARIZONA HEALTH IMPROVEMENT PLAN PANDEMIC RECOVERY AND RESILIENCY WHERE NUTRITION AND PHYSICAL ACTIVITY STRATEGIES ARE BEING LED BY MEMBERS OF THE AZSPAN TEAM. AZSPAN ACTIVITIES WILL LEVERAGE ESTABLISHED PARTNERSHIPS AND SUCCESSFUL LOCAL, EVIDENCE-BASED INITIATIVES TO DOCUMENT PROCESSES AND PROMOTE EXPANSION AND REPLICATION TO OTHER COUNTIES. THIS INCLUDES REPLICATION OF PRODUCE PRESCRIPTION PROGRAMS, EXPANSION OF THE PRODUCE INCENTIVE PROGRAM (DOUBLE UP FOOD BUCKS) TO THE FIVE COUNTIES NOT BEING REACHED BY THE PROGRAM, CREATION OF A COMMUNITY-DRIVEN PARK RX TOOLKIT, IMPLEMENTATION OF SMART GROWTH STRATEGIES, AND THE ESTABLISHMENT OF A 4TH TRIMESTER VILLAGE AND BREASTFEEDING SUPPORT GROUP IN YUMA COUNTY. ACTIVITIES WILL ALSO INCLUDE STATEWIDE TRAINING AND TECHNICAL ASSISTANCE RELATED TO NUTRITION SERVICE GUIDELINES, PHYSICAL ACTIVITY, AND MATERNAL HEALTH AND LACTATION SUPPORT SERVICES TO EMPLOYERS THROUGH THE HEALTHY ARIZONA WORKSITES PROGRAM. STATEWIDE ACTIVITIES IN THE EARLY CHILDHOOD EDUCATION SETTING WILL FOCUS ON REVISED WRITTEN POLICIES AND PROCEDURES FOR THE EMPOWER PROGRAM, INCREASED UTILIZATION OF GO NAPSACC WITH AN EMPHASIS ON FARM 2 ECE, AND IMPLEMENTATION AND ADAPTATION OF THE STATE-SPECIFIC NUTRITION POLICY ALIGNMENT TOOLKIT FOR ECE FACILITIES. AZSPAN TEAM MEMBERS AND PARTNERS HAVE EXPERTISE IN NUTRITION, ACTIVE LIVING STRATEGIES, BREASTFEEDING, AND EARLY CARE AND EDUCATION, AND ARE SKILLED AT USING COMMUNITY-CENTERED, TRAUMA-INFORMED APPROACHES TO POLICY, SYSTEM AND ENVIRONMENTAL CHANGES. AZDHS IS WELL-POSITIONED TO QUICKLY IMPLEMENT PROJECT ACTIVITIES TO IMPROVE NUTRITION, INCREASE EQUITABLE ACCESS TO SAFE PHYSICAL ACTIVITY, INCREASE BREASTFEEDING AND SUPPORT HEALTH IN EARLY CARE AND EDUCATION PROGRAMS FOR ALL ARIZONANS.

THE HUALAPAI RESERVATION (> 1 MILLION ACRES) IS HOME TO APPROXIMATELY 1650 RESIDENTS (1063 ADULTS), WHO LIVE IN OR NEAR THE ONLY RESIDENTIAL AND COMMERCIAL CENTER, PEACH SPRINGS, AZ. TRIBAL ENROLLMENT IS APPROXIMATELY 3300; AN ESTIMATED 50% OF ENROLLED MEMBERS LIVE OFF RESERVATION BUT OFTEN WORK IN PEACH SPRINGS SO HAVE ACCESS TO HSDPI ACTIVITIES. MEDICALLY, THE COMMUNITY IS SERVED BY THE IHS PEACH SPRINGS HEALTH CENTER (PSHC) IN THE COLORADO RIVER SERVICE UNIT (CRSU). PHSC REPORTS A USER POPULATION OF 3000. PHSC IS AN OUTPATIENT CLINIC PROVIDING PRIMARY CARE (3-4 PROVIDERS), PHARMACOLOGICAL SERVICES AND PUBLIC HEALTH NURSING. MANY SPECIALTY SERVICES, E.G., DENTISTRY, PODIATRY, KIDNEY DIALYSIS AND OPTHALMOLOGY ARE INCONSISTENT OR REQUIRE TRAVEL TO CONTRACT SERVICES IN KINGMAN, AZ (55 MI ONE WAY TO THE WEST) OR CRSU (150 MI ONE WAY TO THE SOUTHWEST). THE HUALAPAI TRIBE MANAGES EMERGENCY MEDICAL SERVICES PROVIDING URGENT CARE AND TRANSPORT OFF RESERVATION. HELICOPTER EMERGENCY SERVICES ARE CONTRACTED BY THE TRIBE AND AVAILABLE ON AN AS NEEDED BASIS. PSHC HAS STRUGGLED TO MAINTAIN CONSISTENT PROVIDERS. MANY PATIENTS DO NOT HAVE A LONG-TERM RELATIONSHIP WITH A PROVIDER AND SUBSEQUENTLY MAY HESITATE TO SEEK HEALTH CARE UNLESS CONFRONTED WITH AN EMERGENCY. THE EDUCATION, ACTIVITIES AND RELATIONSHIPS PROVIDED BY THE HSDPI STAFF AND PROGRAM ARE CRITICAL IN THIS ENVIRONMENT OF UNCERTAIN PATIENT-PROVIDER RELATIONSHIPS. HSDPI'S BEST PRACTICE IS DIABETES RELATED EDUCATION; THE STAFF ARE WELL TRAINED AND EXPERIENCED IN PROVIDING LOCALLY AND CULTURALLY RELEVANT INSTRUCTION AND MENTORING. HSPI'S TARGET POPULATION IS ADULTS WITH A BODY MASS INDEX (BMI) GREATER THAN 25, AN ESTIMATED 96% OR 1020 ADULTS. HSPI PROPOSES TO ENROLL 15% OF THE ELIGIBLE ADULTS OR 153 PARTICIPANTS. THE TRIBAL MEMBERSHIP INCLUDES BOTH THE AKIMEL O’ODHAM (PIMA) AND PEE-POSH (MARICOPA) PEOPLE.
