THIS AGREEMENT PROVIDES FULL FEDERAL FUNDING IN THE AMOUNT OF $522,170. ACTIVITIES:THE ACTIVITIES TO BE PERFORMED INCLUDE TRAINING STAFF, INVASIVE SPECIES REMOVAL, CONDUCT WATER QUALITY MONITORING, COMMUNITY OUTREACH AND EDUCATION, CONDUCT PESTICIDES INSPECTIONS, AND ENSURE COMMUNITY IS RECYCLING CORRECTLY.SUBRECIPIENT:NO SUBAWARDS ARE INCLUDED IN THIS ASSISTANCE AGREEMENT.OUTCOMES:THE ANTICIPATED DELIVERABLES INCLUDE DESCRIPTIONS OF TRAINING BENEFITS, BEFORE/AFTER PICTURES OF WHERE INVASIVE SPECIES WERE REMOVED, WATER QUALITY MONITORING DATA SUBMISSION, OUTREACH AND EDUCATION MATERIALS AS WELL AS DESCRIPTIONS OF ACTIVITIES, PESTICIDE INSPECTION REPORTS AND DESCRIPTIONS OF WHAT WILL BE DONE TO CONTINUE TO IMPROVE THE COMMUNITY'S RECYCLING PROGRAM. AK-CHIN DIABETES PREVENTION AND MANAGEMENT PROJECT - AMERICAN INDIAN/ALASKA NATIVE (AI/AN) PEOPLE ARE DISPROPORTIONATELY AFFECTED BY DIABETES. IN 2018-2019, 14.5% OF AI/AN PEOPLE AGED 18 YEARS OR OLDER HAD DIAGNOSED DIABETES COMPARED TO 7.4% OF NON-HISPANIC WHITE PEOPLE. THIS IS COMPARATIVE TO THE RATES WITHIN THE AK-CHIN INDIAN COMMUNITY. AS OF 2020 THE RATES ARE ESTIMATED AT OVER 13% DIAGNOSED WITH DIABETES. IN ADDITION, AI/AN PEOPLE HAVE HIGHER RATES OF DIABETES-RELATED MORBIDITY AND MORTALITY THAN THE GENERAL U.S. POPULATION. THE AK-CHIN DIABETES PROJECT WILL FOCUS ON PHYSICAL ACTIVITY EDUCATION KEYING ON DIFFERENT VARIABLES TO GET THE COMMUNITY MOVING RANGING FROM ONE ON ONE GUIDANCE, TO COMMUNITY EVENTS, AND GROUP CLASSES AND CURRICULUM EDUCATION. THERE WILL ALSO BE FOCUS ON COMMUNITY WELLNESS OUTREACH INCLUDING BUT NOT LIMITED TO EVIDENCE BASED CLASSROOM STYLE CLASSES AND COMMUNITY WELLNESS EDUCATION ARTICLES. THE COMMUNITY HAS A YOUNG DEMOGRAPHIC AND IT IS IMPORTANT CREATE A POSITIVE WELLNESS ENVIRONMENT FOR THE FUTURE. THE AK-CHIN DIABETES PROJECT WILL CONTINUE TO HAVE A FOCUS ON THE YOUNG PEOPLE AS WE HAVE SINCE THE PROGRAM'S INCEPTION IN 2001. IN ADDITION, IT IS IMPORTANT TO MAINTAIN AND IMPROVE THE ELDER’S QUALITY OF LIFE AND PREVENT POSSIBLE DEBILITATING DISEASE COMPLICATIONS. AK-CHIN TRIBAL TRANSIT HAS BEEN OFFERING A FIXED CIRCULAR ROUTE FOR RESIDENTS THAT PROVIDES TRANSPORTATION WITHIN AK-CHIN AND THE NEIGHBORING COMMUNITY OF MARICOPA. IN ADDITION WE ALSO PROVIDE ON-DEMAND CURB TO CURB SERVICES AS WELL.ACTIVITIES TO BE PERFORMED: LOW LINE BUS - AK-CHIN TRIBAL TRANSIT CURRENTLY HAS A FLEET OF SIX FOURTEEN-PASSENGER BUSES AND DOES NOT HAVE A PASSENGER BUS CAPABLE OF PROVIDING APPROPRIATE AND SAFE MOBILE SERVICES TO PASSENGERS THAT ARE SEMI-AMBULATORY. OUR FLEET CAN ACCOMMODATE AMBULATORY AND NON-AMBULATORY PASSENGERS. WE ARE IN CRITICAL NEED FOR A PASSENGER VEHICLE THAT CAN ACCOMMODATE OUR SENIOR CITIZENS AND THOSE WITH LIMITED PHYSICAL MOBILITY WHICH IS FOCUSING ON OUR SEMI-AMBULATORY POPULATION. OUR SEMI AMBULATORY POPULATION IS 21% WE CURRENTLY TRANSPORT 3-4%. TRANSIT WOULD LIKE TO PURCHASE A LOW LINE MOBILITY BUS. THE FRONT OF THE BUS WILL HAVE A RAMP FOR SAFE ACCESSIBILITY FOR PASSENGERS THAT CANNOT ASCEND OR DESCEND STAIRS. THIS BUS WILL BE INCORPORATED INTO OUR CURRENT OPERATING STRUCTURE WITH THE REST OF OUR TRANSIT FLEET AND COMPLEMENT AND ENHANCE OUR CURRENT SERVICES. A LOW LINE BUS IS ESSENTIAL IN OUR COMMUNITY. MOST OF OUR ELDERLY POPULATION IN THE COMMUNITY FALL UNDER THE CATEGORY OF LOW TO MODERATE INCOME. TRANSPORTATION OPTIONS ARE LIMITED AND THE CURRENT CLOSEST BUSINESS THAT PROVIDES THIS SERVICE IS IN EXCESS OF THIRTY MILES AWAY ONE WAY. ROUTE SIGNAGE - AK-CHIN TRIBAL TRANSIT WAS INCORPORATED IN 2016. FIXED ROUTES IN ADDITION TO ON-DEMAND SERVICES ARE OFFERED TO AK-CHIN INDIAN COMMUNITY AND THE SURROUNDING COMMUNITY OF MARICOPA. AFTER CAREFUL REVIEW OF SURVEYS AND COMMUNITY FEEDBACK WE HAVE RECOGNIZED OUR PRESENCE AT MANY LOCATIONS IS CONFUSING TO OUR COMMUNITY AND UNCLEAR. AK-CHIN TRIBAL TRANSIT IS CURRENTLY VISITING STOPS AT LOCATIONS THAT HAVE NO SIGNAGE DESIGNATION OR NUMBERS ASSOCIATED WITH THEM. AK-CHIN TRIBAL TRANSIT WOULD LIKE TO INSTALL THIRTY-THREE SIGNS PLACED AROUND THE COMMUNITY TO REMEDY THESE CONCERNS AND ENCOURAGE RIDERSHIP TO THRIVE. RADIO EQUIPMENT- OUR MOBILE AND STATIONARY EQUIPMENT IS IN FAIR OPERATING CONDITION AND NEEDS TO BE REPLACED. OF THE FIVE REMAINING FUNCTIONING UNITS TWO ARE AT LIMITED LIFE REMAINING DUE TO EXTENSIVE USE AND THOSE BATTERIES FUNCTION AT APPROXIMATELY 25% OF LIFE. OUR MODEL RADIOS HAVE BEEN PHASED OUT AND REPLACEMENT EQUIPMENT IS UNOBTAINABLE. TWO OTHER UNITS IN THE BUSES ALSO SHOW SIGNS OF WEAR AND ARE IN FAIR CONDITION DUE TO USE AND BEING MOUNTED IN FULL SUN. WE EXPERIENCE INTERMITTENT RADIO TRANSMISSIONS AND LONG WAIT TIMES BEFORE A SIGNAL PICKS UP AGAIN TO ALLOW US TO TRANSMIT. AFTER EVERY TRANSMISSION WE MUST WAIT 8-15 SECONDS BEFORE THE RADIO CAN BE TRANSMITTED ON. CONTINUOUS TRANSMISSIONS ARE DIFFICULT TO ACCOMPLISH. FULL SUPPORT FROM OUR COMMUNITY COUNCIL WAS OBTAINED TO MOVE FORWARD IN RECTIFYING OUR CURRENT TRANSIT COMMUNICATION SETBACKS. OUR CURRENT SETBACKS INCLUDE LIMITED HANDHELD RADIO EQUIPMENT TO EFFICIENTLY PROVIDE SERVICES. THE TRANSIT DEPARTMENT IS CURRENTLY OPERATING AT 50% DUE TO A MOBILE RADIO SHORTAGE. OF SIX BUSES FOUR BUSES CURRENTLY DO NOT HAVE RADIO COMMUNICATIONS EQUIPMENT. TRANSIT UTILIZES MOBILE RADIOS IN THESE VEHICLES AND AS A RESULT OF THE SHORTAGE DRIVERS ARE USING MOBILE PHONES WHEN STOPPED AND IN A SAFE LOCATION. PENDING FULL REPLACEMENT TRANSIT HAS COORDINATED WITH LOCAL DEPARTMENTS FOR TEMPORARY ASSISTANCE AND USE OF THEIR EQUIPMENT. THEIR PARTICIPATION HAS LED TO PROLONGING THE LIFE OF OUR EQUIPMENT. BULLETIN BOARDS - FIVE YEARS AGO BUS STOP SHELTERS WERE CONSTRUCTED IN SEVENTEEN LOCATIONS IN THE COMMUNITY. ONE OF THE AMENITIES PROVIDED AT THE SHELTER WAS A CLEAR SWING-FRAME BULLETIN BOARD. THE REPLACEMENT OF THESE BOARDS IS IDEAL FOR THE CONTINUED SUPPORT TO THOSE WHO HAVE NO ACCESS TO TECHNOLOGY. WITHOUT REPLACEMENT TRANSIT IS UNABLE TO CONVEY TO THE PUBLIC ROUTE INFORMATION EVENTS SERVICES MEETINGS AND CDC GUIDANCE. BUS SHADE STRUCTURES - AK-CHIN TRIBAL TRANSIT -CHIN TRIBAL TRANSIT IS SEEKING A SHADE STRUCTURE FOR OUR FLEET OF VEHICLES. THIS SHADE WILL SERVE SEVERAL PURPOSES IN OUR ARIZONA DESERT CLIMATE. IT WILL PROTECT OUR ASSETS HELP TO SLOW THE PREMATURE AGING OF BUS EQUIPMENT AND PROVIDE COMFORT FOR STAFF AND PASSENGERS. EXPECTED OUTCOMES: LOW LINE BUS - UPON RECEIPT OF FUNDING AK-CHIN TRIBAL TRANSIT WILL START THE PURCHASING PROCESS. THIS BUS WILL BE EQUIPPED SPECIFIC TO OUR NEEDS. BUS SHALL BE OBTAINED AND PLACED INTO SERVICE WITHIN SIX TO EIGHT WEEKS AFTER PURCHASE AND INSTALLATION OF OPERATING EQUIPMENT.ROUTE SIGNAGE - CULTURAL AND ENVIRONMENTAL CLEARANCES HAVE TAKEN PLACE AND ARE ALMOST COMPLETE. ROUTE SIGNAGE - THE INSTALLATION OF SIGNAGE WE PREDICT WILL OPEN UP THE OPPORTUNITY FOR INCREASED RIDERSHIP AND ATTENDANCE ON THE ROUTES DUE TO FOOT TRAFFIC AT IDENTIFIED LOCATIONS. UNDERSTANDING OF KNOWN LOCATIONS WILL ASSIST THE PASSENGERS IN KNOWING THEY ARE WAITING IN THE APPROPRIATE LOCATION. ROUTE STOPS CAN THEN BE IDENTIFIED WITH A NUMBER. THE PANDEMIC HAS AFFECTED OUR RIDERSHIP DUE TO CDC GUIDELINES AND SOCIAL DISTANCING HOWEVER OUR ANNUAL RIDERSHIP FROM JULY OF 2020 TO JULY 2021 WAS 5275 PASSENGERS. SOME RESIDENTS INFORM US OF THEIR INTENT TO BOARD AT A SPECIFIC LOCATION FOR THE ROUTE AND UPON ARRIVAL THE PASSENGER IS NOT THERE. WE FEEL THAT SIGNAGE WILL HELP TO ELIMINATE THESE EVENTS.RADIO EQUIPMENT - THE REPLACEMENT OF EQUIPMENT WILL IMPROVE OUR SERVICE OPERATIONS. TIMELY TRANSMISSION VIA RADIO REGARDING CURRENT AND POTENTIAL PASSENGERS WILL BE MORE EFFICIENT FOR OUR SERVICE OPERATIONS. DRIVER-TO-DIVER COMMUNICATION AS WELL AS CUSTOMER SERVICE WOULD BOTH BE GREATLY IMPROVED WITH AN UPDATED COMMUNICATION SYSTEM. THE FOLLOWING YEAR IN 2020, ACIC ASSUMED MANAGEMENT OF THE HEALTH EDUCATION PROGRAM AS A NEW PSFA; AND EFFECTIVE JANUARY 1, 2022, THE ACIC FURTHER EXPANDED AND ASSUMED OPERATION OF THE PURCHASED AND REFERRED CARE (PRC) PROGRAM FOR ACIC COMMUNITY MEMBERS. THE REMAINDER OF DIRECT HEALTH CARE SERVICES ARE PROVIDED TO ACIC UNDER THE GILA RIVER INDIAN COMMUNITY SELF-GOVERNANCE MULTI-YEAR AGREEMENT. AS WE PLAN FOR ASSUMPTION OF NEW PSFAS, THE NEED FOR A THOROUGH REVIEW OF INTERNAL HEALTH OPERATIONS IS NOW TIMELY AND NECESSARY TO MANAGE AND ORGANIZE THESE PSFAS. UNDER THIS TMG, ACIC INTENDS TO CAREFULLY EXAMINE AND EXPLORE HEALTH MANAGEMENT OPTIONS INCLUDING THE ESTABLISHMENT OF A HEALTH BOARD, HEALTH DEPARTMENT ORGANIZATION, AND FINANCIAL AND DATA SYSTEMS IN ORDER TO STRENGTHEN AND IMPROVE INTERNAL CONTROLS AND CAPABILITIES. THE REQUESTED FUNDING UNDER THIS TMG APPLICATION WILL ALLOW THE ACIC COMMUNITY COUNCIL TO TAKE A MORE COMPREHENSIVE APPROACH IN DETERMINING THE BEST HEALTH CARE STRUCTURE FOR THE TRIBAL GOVERNMENT. THESE ACTIVITIES ARE TO IMPROVE AND MAINTAIN THE TRIBE'S AIR QUALITY.ACTIVITIES:THE RECIPIENT WILL IMPLEMENT ACTIVITIES WITHIN THE SCOPE OF THE SECTION 103 PROGRAM INCLUDING: STRATEGIC PLANNING AND EVALUATION, COMPLIANCE ASSISTANCE, MONITORING RESERVATION-WIDE AIR EMISSIONS, DEVELOPING AN AIR REGULATORY PROGRAM, PROVIDING COMMUNITY EDUCATION AND OUTREACH, AND PARTICIPATING IN REGIONAL AND NATIONAL INITIATIVES TO IMPROVE AIR QUALITY. THESE ACTIVITIES SUPPORT THE TRIBE'S GOAL TO PROTECT THE HEALTH OF THE TRIBAL COMMUNITY. SUBRECIPIENT:NO SUBAWARDS ARE INCLUDED IN THIS ASSISTANCE AGREEMENT.OUTCOMES:OUTCOMES INCLUDE A BROADENED AWARENESS OF AIR QUALITY PROGRAMS THAT ARE ESTABLISHED TO ADDRESS THE ATTAINMENT AND MAINTENANCE OF (NAAQS) AS WELL AS IMMEDIATE AND EMERGING THREATS TO PUBLIC HEALTH AND WELFARE FROM AIR POLLUTION. INTENDED BENEFICIARIES: DIRECT BENEFICIARIES OF THIS PROGRAM INCLUDE AK-CHIN INDIAN COMMUNITY AREA RESIDENTS WHO WILL HAVE DECREASED EXPOSURE TO CRITERIA AND HAZARDOUS AIR POLLUTANTS.

THIS PROPOSED PROJECT WOULD BE PART OF THE DISTRICT S STRATEGY TO UPGRADE THEIR AGING FACILITIES AND SCADA SYSTEM. THE PROPOSED IMPROVEMENTS ARE TO REMOVE THE EXISTING LATERAL WC TURNOUT GATE AND INSTALL A NEW 42 RUBICON SLIPMETER GATE. A COFFERDAM OR PIPE PLUG WILL NEED TO BE INSTALLED AS THE SANTA ROSA CANAL (SRC) CANNOT BE DEWATERED DUE TO CONSTANT DELIVERY TO FARMERS AND THE AK-CHIN INDIAN COMMUNITY (AK-CHIN) DOWNSTREAM OF THIS SRC CHECK STRUCTURE AND TURNOUT, AS WELL AS REQUIRED APPURTENANCES TO MAINTAIN AND OPERATE THE NEW SLIPMETER GATE. THE NEW SLIPMETER GATE WILL ELIMINATE THE SPILLS DUE TO THE GATE LEAKING AND PROVIDE ACCURATE FLOW MEASUREMENT FOR FUTURE SRC LATERAL WC TURNOUT SERVICE.THIS SMALL-SCALE WATER EFFICIENCY PROJECT IS A PRIORITY TO MARICOPA-STANFIELD IRRIGATION AND DRAINAGE DISTRICT AND WAS APPROVED BY OUR BOARD. WE HOPE TO LEVERAGE FUNDING FROM THE WATERSMART PROGRAM TO HELP MARICOPA STANFIELD IRRIGATION AND DRAINAGE DISTRICT COMPLETE THIS MULTIPHASE PROJECT, MEETING BOTH THE DISTRICT S AND RECLAMATION S GOALS TO CONSERVE AND BETTER MANAGE OUR WATER RESOURCES AND INCREASE EFFICIENCY OF OUR SYSTEM. STAKEHOLDERS INCLUDE FARMERS AND CUSTOMERS IN THE DISTRICT, THE CENTRAL ARIZONA IRRIGATION AND DRAINAGE, AK-CHIN, AND THE BUREAU OF RECLAMATION. IN ADDITION TO AK-CHIN, THE DISTRICT BORDERS THE GILA RIVER INDIAN COMMUNITY (GRIC) TO THE NORTHEAST AND THE TOHONO O ODHAM NATION TO THE SOUTH. THE DISTRICT WOULD LIKE TO WORK WITH BOTH NATIVE AMERICAN COMMUNITIES TO BE GOOD NEIGHBORS AND STEWARDS TO PROTECT THE NATURAL ENVIRONMENT THAT WE SHARE ALONG OUR BORDERS.
